Prepayments are required for the use of REVITA’s dietary, medical and treatment services (ONDAMED) before each scheduled visit.

As a result of cancellation 3 days before the scheduled date, we refund the prepayment. In the event of unjustified cancellation of the visit, there is no possibility of a refund.

In the case of resignation later than 3 days before the date of the booked stationary visit, the prepayment is not lost – we postpone the date of the visit after consulting the registration ourselves: tel. 694 456 511.

Stationary visit

Prepayments are required when using the services of REVITA before each scheduled visit.

Prepayment applies to visits to:

the first medical and dietary visit with the BETA / Food Detective test at Dr. Dr. Agnieszka Arendarczyk or a dietitian
dietary consultation with MD Dr. Agnieszka Arendarczyk or a dietitian
the first visit without BETA testing at your dietitian
ONDAMED treatments
